This is what I know if you can add to it I would be most interested to hear.

Yoda - died on dagobah
Yaddle - killed in battle b4 attack of the clones
Mace Windu - killed by sidious
Adi Gallia - killed by general grievous ?
Agen Kolar - killed by sidious
Depa Billada - fell into coma after battle with mace
Ki-Adi Mundi - killed by clone troopers on mygeeto
Plo Koon - killed by clone troopers
Yarael Poof - killed on coruscant (his murder avenged by jango fett)
Oppo Rancisis - killed by sora bulq
Kit Fisto - killed by sidious
Saesee Tinn - killed by sidious

looks like you've got most of them....i gotta think awhile ro see if i can think of any...hmmmmmmmm     good idea for a post though!

oh yeah forgot the one that jango killed on geonosis coleman trebor.

yeah probably got most of the council forget the council then just any jedi lets see how many deaths we can all come up with and how many boba is responsible for.

Dont forget Aayla Secura killed by clone troopers on Felucia.
And all of the lil Padawans killed by Anikin.... :D....he,he,he..

Even Piell  Killed at the Jedi Massacre in ROTS????
Stass Allie (was she in the council?) killed by Clones
Eeth Koth  Killed in the Clone Wars? (that is I think he was in AOTC if not he must've been killed before EP2 and yes I know I might've had him confused with Agen Kolar, there is no need to point it out...)
Shaak Ti Killed By Grievous (ROTS deleted scenes)

Well He Survived Order 66 (with thanks to Villie and some Wookies) and escaped Kashykk, And fathered a Child, it's kinda unclear what happens to him next.

Although... He did meet Han Solo on some Planet in Star Wars Tales.
He was being chased by Bounty Hunters. Whom He killed.

But no matter how plausible this story is, Tales is not necessarily Canon...

It was Star Wars Tales 11 for the Appearance with Solo.

You can also get it in Star Wars Tales Volume 3, It's got a really cool story with Fett Vs. Vader. And Fett has a Lightaber. Plus its got an Aurra Sing Story....

Star Wars Insider #87 has an article about four pages long about a Casualty Report on Order 66.  It talks about how they died and who killed them.  It also says the reason why they killed them.  The Emperor doesn't tell the public that the troops killed the Jedi because of Order 66, it was because of other reasons.  You have to read it.
